Hello and Welcome to Ridge Road Elementary School


My name is Jennifer Biehn, RN, I am the school nurse here at Ridge Road Elementary. I have been providing health services since August of 2023 at Ridge, and have been working in the North Haven School district since 2018. I am so excited to meet all of our new students and families for the 2024-2025 school year! Below I have provided a list of information That I will need before your child starts the 2024-2025 school year. This information can be uploaded in PowerSchool. If you have trouble uploading the information a hard copy is also acceptable. If you have any medical concerns about your child please feel free to give me a call or stop by the health office. The more information I have about your child the better I am able to support their wellbeing while here at school.


All Students will need:

  • A current Physical Exam is required to be performed during the year prior to the 1sr day of Kindergarten.
  • This should be on the CT  “blue” School Aged Health Assessment Form please note the front page should be filled out by a parent or guardian and requires your signature.
  • Also up to date Immunization Record as per the CT State Immunization requirements for the school year 2024-2025.

Religious exemptions for vaccinations are no longer accepted by the State of Connecticut unless you had a religious exception on file prior to April 27th, 2021


If your Child has Asthma, Food Allergies or Seizure Disorders:

  • Asthma Action Plan and Medication Administration Orders for Rescue Inhaler
  • Allergy Action Plan and Medication Administration Orders for Epinephrine and if needed Benadryl
  • Seizure Action Plan and Medication Administration Orders for Rescue Medication
  • Allergy and Seizure Individualized Health Care Plans need to be filled out in PowerSchool yearly-this is found under the Forms section of PowerSchool

If your Child needs to take Medication during the school day:

  • Medication Administration Orders signed by your child's doctor and also yourself for administration.
  • Medication Provided in the original packaging with Order Administration Label attached *Most daily medications or twice a day medications should be given at home prior to school or after school hours.

Please note: According to CT State law a parent must transport medications to the school and then pick them up at the end of the school year.


Travel:

If you plan to travel outside of the country, please notify the nurse prior to travel for information! 


Contact Information: please be sure to have at least 2 other family or friends for the school to contact in the event that we are unable to reach you as the 1st emergency contact person. This information is located under the Forms section of PowerSchool.
